Email the completed form to sales@camerarailsystems.com. Photos or videos of the fault can be attached if useful.
We will review the request and send you the RMA reference number, shipping address and any specific instructions.
Only after receiving our confirmation, pack the equipment securely and include a copy of the completed form in the shipment.
The form records the customer reference, product details, serial number, fault description, items included in the shipment and repair authorization. The second page is used as our internal service worksheet.
Product / part name, P/N, serial number, quantity, detailed fault description and a list of all accessories included in the shipment.
The shipping address is intentionally not published. It is provided by email after the RMA request has been reviewed and accepted.
You can request an estimate before repair or authorize work up to a specified amount directly on the form.
Send us the product code, serial number and a short description of the problem before shipping anything.
